Its simply that your CPU/GPU cannot keep up with the data, and the game is haphazardly assigning things to load all over the place, at any distance. When it works as it was designed, it looks quite nice! Whenever textures pop in, that's an error in the game's rendering engine and has nothing to do with your draw distance. Just to clarify - objects and NPCs are supposed to fade in at a set distance, determined by your detail settings.
